Job Summary
Initiate outreach efforts to households at risk of homelessness due to financial hardship or eviction, conducting program education and assisting with FFA online applications. Assess housing barriers and complete prioritization tools to determine vulnerability, then rapidly collect documentation to verify eligibility for one-time or short-term financial assistance. Collaborate with the Central Fiscal Agent and Right to Counsel program to submit requests and legal referrals, while developing individualized housing stabilization plans linked to employment, legal, and community services. Provide advocacy with landlords, create communication channels between parties, and facilitate workflows with legal service entities. Document all services, referrals, and participant feedback, and assist individuals in obtaining benefits, medical services, and credit counseling.
Required Qualifications
- High School Diploma or GED/Equivalent
- Minimum of one (1) year case management or customer service experience
- Able to communicate and interact with subcontractors, service providers and community partners in a culturally competent manner
- Valid CA Drivers License and use of reliable personal vehicle
