Job Summary
Case Manager – Family Preservation position responsible for maintaining an assigned caseload and delivering intensive case management services to families referred by Macomb County DHHS at risk of child removal due to abuse or neglect related to substance use. Responsibilities include conducting screenings and comprehensive assessments for substance use disorders, implementing and monitoring person-centered/family-centered treatment plans, identifying community resources, facilitating monthly education workshops on substance use prevention, applying motivational interviewing and other engagement strategies, completing documentation per funding-source standards, delivering services in the community, and coordinating with agencies; hours may include evenings and weekends to meet community needs.
Required Qualifications
- A bachelor’s degree in social work, sociology, psychology, family ecology, consumer/community services, family studies, family and/or child development, guidance/school counseling, counseling psychology or criminal justice.
- Experience working with families/adults with substance use disorders or related field.
- Completion of ICHAT check, National and State Sex Offender Registry Check, Central Registry Check, and Children’s Protective Services background check.
- Ability to work independently as well as in a team environment.
