Job Summary
Case Manager at Exalt Health Rehabilitation Hospital in Wildwood, FL, collaborates with physicians to manage psychosocial assessments and individualized care plans, coordinates resources and post-discharge support, and facilitates discharge planning and insurance authorizations to ensure timely, cost-effective rehabilitation for patients with complex conditions such as stroke, brain injury, spinal cord injury, and orthopedic injuries. Responsibilities include interdisciplinary team participation, documentation of assessments and progress, and advocating for patients and families throughout the continuum of care.
