Job Summary
Case Manager for the Encampment Resolution Funding (ERF) program providing outreach and field-based case management to unsheltered individuals in San Mateo County CHEZ sites, with a caseload of 15–20 clients, development of client-centered housing plans based on Housing First principles, rapid response outreach within 8 business hours, active MDT participation with stakeholders, trauma-informed and harm-reduction services, assistance with housing applications and referrals for services (substance use treatment, mental health care, and permanent supportive housing), warm handoffs and continuity of care across programs, and ongoing post-housing support for up to a year.
Required Qualifications
- Bachelor’s degree in social work, psychology or related field
- minimum of two years’ case management experience
- Excellent communication and interpersonal skills
- Bi-lingual Spanish language capabilities a plus
