Job Summary
Manage a caseload of approximately 35 participants by performing Individual Service Needs Assessments and collaborating to develop housing-focused Individual Service Plans. Implement these plans through direct meetings, track progress, and provide linkage services for housing, income, healthcare, and behavioral health resources. Maintain accurate electronic records of assessments, crisis management, and discharge services while adhering to state, county, and federal regulations. Report participant progress to Program Leadership and assist with planned exits to appropriate services. This fully onsite role requires consistent in-person engagement with clients at ECS program locations.
Required Qualifications
- High School Diploma + 4 years' experience or AA degree in similar field + 3 years' experience, or CHW Certificate/CADC + 2 – 4 years' experience, or BA/BS degree in similar field + 2 – 4 years' experience, or MA/MS degree in similar field + 1 year experience
- Associate degree in psychology, counseling, social work, or another related field and 3 years of relevant work experience
- CCSF Community Health Worker Certificate and 2 – 4 years of relevant work experience
- High school diploma/GED and 4 years of relevant work experience
- Experience working in integrated service teams using person-centered harm-reduction and trauma-informed approaches
- Background working with unhoused individuals
- Effective communication skills (written and oral)
- Excellent organizational and time management skills
- Strong administrative and organizational skills
- Must possess excellent charting, recordkeeping, and professional writing skills
- Knowledge of Microsoft Office programs and experience documenting services into electronic record systems
- Must be able to problem solve and quickly assess/respond to a variety of situations, including medical emergencies, psychiatric emergencies, conflict resolution and de-escalation interventions, as well as other types of crises
- Operate with cultural humility in a diverse program and staff population
- Possess a knowledge of and sensitivity to adults with substance abuse, domestic violence, incarceration, HIV/AIDS, mental health including co-occurring disorders, and history of trauma
- Must secure fingerprint image screening and annual TB screening
- Must be able to meet physical requirements of the position, which may include lifting up to 30 lbs
