Job Summary
Coordinate patient care by assisting with discharge planning, arranging referrals, and scheduling follow-up appointments to ensure continuity of treatment. Serve as a patient advocate and liaison for specialty providers and social services while providing health education and translation support. Manage medical records by documenting pertinent data, entering information into electronic systems, and generating routed data forms for billing and medical records. Perform clerical duties including answering telephone calls, filing, and maintaining inventory logs for supplies and immunization materials. Ensure compliance with HIPAA guidelines and clinic safety standards while adhering to daily assignments directed by the supervisor.
Required Qualifications
- Case Management certificate or 1-year experience in related position
- Medical Assistant training and certificate of completion
- Minimum of one year experience in the medical field
- Annual CPR certification – adult, child, infant, and obstructed airway
- Valid California Driver License
- Automobile insurance coverage
- Reliable automobile for use on the job
- TB clearance
- Annual Influenza Vaccination
