Job Summary
Case Manager at Copa Health coordinates care across behavioral health, medical, and dental providers; develops and maintains the Integrated Service Plan (ISP) within required time-frame; engages with service participants and their families to educate and coordinate care; documents progress and supports recovery goals; collaborates with an integrated care team to ensure services prescribed in the ISP are implemented and to meet program and department needs.
Required Qualifications
- High School degree required
- Minimum four years of experience in behavioral health and SMI (serious mental illness) or a Bachelor's degree in a related field with one year of related experience
- Arizona valid Driver’s License with vehicle and state minimum insurance coverage
- Safety sensitive requirements and driving qualifications per Copa Health policy
- Background check and drug testing required (pre-employment)
- Bilingual (English/Spanish) preferred
