Job Summary
Provide comprehensive case management for adults with severe mental illness and co-occurring disorders, delivering services ranging from monthly to weekly contact via in-person or telehealth platforms. Manage client needs including income, housing, insurance, transportation, and activities of daily living while conducting crisis assessments, safety planning, and referrals to legal and social services. Educate clients on money management, symptom reduction, and household maintenance, and gather all required CCBHC data metrics. Work within a multi-disciplinary team using the Certified Community Behavioral Health Clinic model to coordinate whole-person care, physical health screenings, and expanded access to specialized services for complex mental health and substance use challenges.
Required Qualifications
- High school graduate or HS equivalent
- 36 months experience in behavioral health field
- Bachelor's Degree in any field
- 6 months experience in a behavioral health field
- Bachelor's degree in social science field
- Case Management II certification or obtain by taking the first available Case Management Certification course provided by ODMHSAS after hire
- Strong commitment to the right and ability of each person served to live in normal community residences; and have access to helpful, adequate, competent, and continuous supports and services
- Skills and competence to establish supportive trusting relationships with persons with mental illnesses and/or substance abuse issues
- Respect for client rights and personal preferences in treatment
- Valid Oklahoma Driver License
- Satisfactory driving record
- Use personal automobile to travel to locations other than primary office
- Must be able to lift 50 lbs
