Job Summary
Coordinate client transitions from nursing facilities to community living by creating comprehensive service plans and securing affordable housing, In Home Support Services, and community health resources. Maintain accurate documentation for state and program requirements while acting as a liaison between clients, family, landlords, and agencies such as the Housing Authority and Social Security. Facilitate the development of a circle of support and provide cross-coverage for colleagues as needed. Work Monday through Friday, 8:30AM to 5:00PM, with a requirement for a valid driver's license and vehicle access.
Required Qualifications
- Two years of experience working in the field of human services
- Bachelor's degree
- Possession of a valid California driver's license
- A driving record that meets agency standards
- Auto insurance
- Access to a vehicle
Desired Qualifications
- Experience working with programs such as IHSS
- Experience working with Medi-Cal
