Case Manager - Duluth Adult Team

Case Manager for Duluth Adult Team provides integrated mental health case management and Behavioral Health Home Services as part of an Integrated Care Team. Responsibilities include four core elements: assessment, planning, referral, and monitoring/coordination; develop and monitor the Individual Community Support Plan; comprehensive case management and care coordination; facilitate Recovery Team Meetings; health promotion and wellness; client and family support; referral to needed community and social support services; and, where available, referral to Peer Support Services. Position types include Full-time or Part-time (0.6 FTE minimum) with hourly-to-salary progression after training. Requires a bachelor’s degree in behavioral sciences or related fields and at least 2,000 hours of supervised experience in delivering services to adults or children with mental illness, substance use disorder, or emotional disturbance (and associated training in recovery, de-escalation, co-occurring disorders, and psychotropic medications). Must pass MVR check and maintain a valid driver’s license and vehicle insurance.
