Job Summary
Conduct placement assessments, develop Case Management Plans, and monitor participant progress relative to their plans through all program phases. Provide interactive services to STOP participants, make referrals to outside agencies, and collaborate on discharge and aftercare plans. Maintain progress notes, document case files per GEO and CDCR standards, and conduct monthly CMP reviews. Manage individual participant services via referrals and CBP program assignments while ensuring implementation of specific treatment modalities under Clinical Supervisor direction. This role supports the reintegration of individuals into the community through evidence-based rehabilitation services.
Required Qualifications
