Job Summary
Case Manager responsible for assisting DKA clients with non-vocational barriers impacting employment, addressing social determinants of health (e.g., food, housing, healthcare, transportation, legal resources) through individualized support, resource referrals, and community services. Collaborates with the client's Employment Specialist or Job Developer to align non-work needs with employment goals. Requires strong communication, problem-solving, and advocacy skills, with culturally sensitive and trauma-informed support. Onsite, client-facing role with limited work-from-home accommodations approved by CEO. Performs confidential case notes, maintains HIPAA compliance, and coordinates referrals via UniteUs; supports benefits access (SNAP, AHCCCS, Social Security), basic-needs resources, and life skills development; emphasizes professional development and adherence to NASW ethics; uses Microsoft Office and related applications.
Required Qualifications
- High School Diploma or GED
- Associate's degree or higher in social work, behavioral health, or a related field
- Peer or Recovery Support Specialist Certification
- Minimum of one (1) year of industry-related experience
- Must be able to obtain fingerprint clearance within 90 days of hire
- Must be able to pass a background check and pre-employment drug screening
- Current CPR and First Aid certification required or must obtain upon hire
- Maintain compliance with Relias and BHT training requirements
