Job Summary
Assess assigned patient cases to identify case management needs and collaborate with physicians, patients, and families to develop comprehensive discharge plans. Lead coordination of care with interdisciplinary teams, monitoring diagnostic testing, treatments, and procedures to ensure continuity throughout the continuum of care. Screens newly admitted patients within 24 hours, identifies at-risk individuals, and partners with clinical staff to meet specific patient needs while maintaining compliance with Centers for Medicare & Medicaid Services regulations. Actively participate in Interdisciplinary Care Rounds and follow through on identified interventions. Requires one year of recent experience in acute care discharge planning, a Colorado RN or LCSW license, and an Associate's or Bachelor's degree in nursing.
Required Qualifications
- One-year recent experience in acute care hospital Utilization Review, Discharge Planning or equivalent
- Licensed as a Registered Nurse in the state of Colorado or from a valid compact state
- Licensed Social Worker/Licensed Clinical Social Worker in the state of Colorado
- Associate's or Bachelor's degree in nursing
- Must possess, or be able to obtain within 90 days, the computers skills necessary to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc
- Must have working knowledge of the English language, including reading, writing, and speaking English
- Must be HIPAA compliant
