Job Summary
Case Manager – Behavioral Health: Coordinate care for individuals with behavioral health and substance use challenges; develop individualized treatment and service plans; arrange referrals for psychiatric services, therapy, housing, transportation, employment, and social supports; maintain HIPAA-compliant documentation; participate in interdisciplinary team meetings and support transitions of care from hospital to community settings.
Required Qualifications
