Job Summary
The Case Manager for Comunilife Inc.'s Safe Haven II program is responsible for providing comprehensive case management services to clients, conducting assessments, developing service plans, and ensuring services are delivered. The role includes home visits, crisis intervention, and maintaining documentation. Candidates should hold a Bachelor’s Degree, have experience in mental health support, and demonstrate skills in individual and group counseling, with preference for those experienced with homeless populations.
Required Qualifications
- Bachelor's Degree
- Minimum two years field work experience
- Direct mental illness work experience
- Individual, family and group counseling experience
Desired Qualifications
- Experience working with homeless individuals in Congregate Programs
- Bilingual in Spanish
- Bicultural knowledge
Additional Requirements
