Job Summary
Case Manager at DeCoach Recovery Centre in Xenia, OH, providing direct client services and linkage to resources for adults with substance use and mental health disorders. Responsibilities include case management under clinical supervision, community-based services, use of a company vehicle, development of daily living skills, advocacy, coordination of care and adherence to the client’s Individual Treatment Plan, collaboration with the Treatment Team, and ongoing quality improvement. Requires effective communication skills and knowledge of behavioral health practices, documentation procedures, and current therapeutic methods. Flex hours are available to best serve clients.
Required Qualifications
- Graduate from an accredited college/university or High School
- Minimum of one year experience in a behavioral health or related field
- Active Ohio Driver's license in the state of Ohio and a clean driving record
