Job Summary
Join Pinnacle Services as a Case Manager focused on supporting individuals with developmental disabilities, serious and persistent mental illnesses, or chemical dependencies. This full-time position involves assessing client needs, coordinating resources, monitoring service delivery, and working closely with interdisciplinary teams.
Required Qualifications
- Bachelor's degree in social work, psychology, sociology, or related field
- One year of experience as a social worker/case manager/care coordinator in a public or private social service agency
- One year of experience in education or treatment of persons served with developmental disabilities or related conditions
- Valid driver’s license and acceptable driving record
- Vehicle covered with current insurance
- Successful clearance of background checks
Desired Qualifications
- Possess a LSW or higher form of BOSW Licensure
- DD Waiver Specific - Bilingual in Spanish
