Job Summary
Case Manager responsible for meeting with PRP program guests at least weekly to provide Christ-centered counseling, monitor progress, and ensure program phase fulfillment. Responsibilities include referring guests to area agencies, coordinating with Operations Manager and Case Management team to determine length of stay, teaching PRP classes, communicating guest welfare to key staff, leading guests from a Christian perspective to encourage positive change, identifying gaps in services, coordinating with staff to set goals for the 4-Phase Transitional Living Program, being on-call for emergencies, coordinating drug and alcohol testing when needed, logging and filing records securely, assisting guests with money management, reporting maintenance concerns, and performing other duties as assigned. Requires adherence to Crossroads’ faith values, strong moral standards, confidentiality, and proficient computer skills. Entry-level position based in Hastings, NE, with in-person duties.
Required Qualifications
- High School diploma or equivalent
- Willingness to obtain CPR and First Aid Certification or hold current CPR/First Aid Certification
- Strong interpersonal and communication skills
- Ability to maintain confidentiality
- Ability to log and file program records securely
- Computer skills and willingness to learn Crossroads Mission Avenue software
- Ability to diffuse conflict and treat guests with dignity and respect
Desired Qualifications

- CPR and First Aid Certification (or willingness to become certified)
