Job Summary
Collaborate with county workers, service providers, and families to coordinate youth treatment, resolve concerns, and advocate for the youth's best interests. Communicate clinical goals with clinicians and maintain congruent services across agency departments while managing paperwork and attending required meetings. Provide direct support to families for visitation and home assessments, adhering to Therapeutic Crisis Intervention and Boys Town Psychoeducational Model principles. Maintain accurate documentation, reports, and records in compliance with agency and state policies. Work full-time with occasional weekend hours and pre-authorized overtime, using a personal vehicle for case management travel.
Required Qualifications
- Bachelor's degree in Social Work, behavioral sciences or a related field
- Associate's degree with 3 years case management experience
- Valid driver's license and auto insurance, free from major infractions and deemed acceptable to our insurance carrier
- Ability to lift 25 pounds without assistance
- Ability to sit, squat, crouch and stand for elongated periods of time
- Ability to lift arms over head in upward reaching motion
- Ability to walk up and downstairs up to several times per day
- Ability to use systems including but not limited to Word, Excel, Outlook, Teams
- Varying levels of travel as determined by departmental needs
- Regular and timely attendance
- Ability to work independently and without direct oversight
- Ability and willingness to take initiative
- Strengths based and client centered
- Experience and/or familiarity with mental health related community-based supports
- Must be able to lift 25 pounds without assistance
