Job Summary
Conduct phone triage and field outreach for individuals in crisis within Tulsa and surrounding areas. Collaborate with the Alternative Response Team to provide service navigation, client advocacy, and follow-up for those assessed by ART. Deliver crisis services designed to divert clients from inpatient hospitalization, jail, homelessness, or emergency departments. This role requires 6+ months of experience with SMI clients, a Bachelor's degree in a social science field, and 2+ years in behavioral health case management. You must hold a valid Oklahoma Driver's License and use a personal automobile for travel. Work within the Certified Community Behavioral Health Clinic (CCBHC) model to coordinate whole-person care for adults and children facing complex mental health and substance use challenges.
Required Qualifications
- 6+ months of experience working with SMI clients
- Bachelor's degree in social science field
- CMII certification
- Strong commitment to the right and ability of each person served to live in normal community residences
- Access to helpful, adequate, competent, and continuous supports and services
- Skills and competence to establish supportive trusting relationships with persons with mental illnesses and/or substance abuse issues
- Respect for client rights and personal preferences in treatment
- Valid Oklahoma Driver License
- Satisfactory driving record
- Use personal automobile to travel to locations other than primary office
- 2 or more years of behavioral health experience as Case manager or other related role
