Job Summary
Full-time onsite Case Manager for Aurora Mental Health & Recovery Connect to Care. Provides trauma-informed case management to individuals with mental health and/or substance use challenges, assisting with initial assessments, crisis intervention, and ongoing engagement in care. Works alongside therapists and care coordination staff to help clients access resources, navigate systems of care, and move toward stability. Responsibilities include facilitating daily living skills, supporting access to employment opportunities or vocational services, coordinating transportation, and educating families and other stakeholders to maximize client outcomes.
Required Qualifications
- Bachelors degree in psychology, social work or other related human services degree
- Or Bachelor's Degree in unrelated field with at least one year of behavioral health experience
- Preferred: Bilingual
- Required Vaccination and TB Test
