Case Manager

Provide ongoing linkage, monitoring, planning, evaluation, and advocacy for clients within the medical, social service, law enforcement, and health department communities. Safely transport clients to related appointments and teach daily living activities including shopping, nutritious meal preparation, and financial budgeting. Monitor symptoms, de-escalate behaviors, and respond to crises involving medical and community professionals as needed. Support clients with mental illness to increase functioning, reduce hospitalizations, and ensure access to community resources.
