Job Summary
Provide case management services including service planning, medication monitoring, and assisting clients with referrals to clinical, vocational, and community resources. Conduct weekly unit visits and monthly inspections while resolving landlord lease issues and coordinating with the public housing authority. Deliver individual counseling and support services, ensuring all documentation meets funding requirements and agency quality assurance standards. Participate in weekly supervision, complete mandatory trainings, and perform crisis intervention. Maintain current knowledge of community resources to help clients access entitlements and maintain housing stability.
Required Qualifications
- Bachelor's Degree in Psychology, Rehabilitation, Social Work or other related mental health discipline
- 2 years experience working with persons with histories of substance abuse and/or mental illness
- Previous work with Veterans
- good verbal and written communication skills
- the ability to make sound judgments regarding client care
