Job Summary
Provide case management services including service planning, medication monitoring, and assistance with referrals to clinical, vocational, and community resources. Deliver individual counseling and support while conducting weekly unit visits and monthly inspections. Manage housing services such as VASH voucher maintenance, life skills education, and lease resolution. Complete all documentation for compliance and quality assurance, participate in supervision and staff meetings, and perform crisis intervention. Maintain current knowledge of agency compliance documents and trainings. This role supports Veterans at Liberty Village with a focus on community integration and advocacy.
Required Qualifications
- Bachelor's Degree in Psychology, Rehabilitation, Social Work or other related mental health discipline
- 2 years experience working with persons with histories of substance abuse and/or mental illness
- Previous work with Veterans
- Good verbal and written communication skills
- Ability to make sound judgments regarding client care
