Job Summary
Provides intake and case management services, along with community support including transportation via program van and life-skills instruction; maintains confidentiality and ethical standards; coordinates resources, referrals, and documentation for Marketplace Apartments clients; conducts intake assessments, HMIS data entry, and supports clients to access housing, benefits, and services; collaborates with nurses, peers, and stakeholders; ensures trauma-informed, client-centered plans and pursuit of outcomes; manages records, reports, and program operations; requires valid NYS driver’s license and two years of relevant experience; bachelor’s degree preferred.
Required Qualifications
- Bachelor’s Degree in Social Work, Human Services, Criminal Justice or related field preferred.
