Job Summary
Assess and document client symptoms, diagnoses, and interventions to open cases and establish individualized treatment plans with measurable goals. Complete functional assessments and service plans within 30 days of case opening and every six months thereafter. Link clients with necessary community resources, monitor intervention effectiveness, and advocate for clients at staff meetings and court hearings. Provide direct clinical services using solution-oriented strategies, including crisis intervention for nighttime and weekend hours as requested. Maintain continuity of care in accordance with Florida Statutes and administrative codes, ensuring at least one monthly face-to-face contact and one bi-monthly home visit per client.
Required Qualifications
- Bachelor's degree required from an accredited college or university in relevant field
- Minimum 1 year of experience providing at least 12 months of related case management services to either the adult or child population
- Completion of AHCA approved targeted case management training within 3 months of hire
- Must be certified through the Florida Certification Board (FCB) within 6 months of hire
- Must be 21 years of age
- Must have a negative drug screening prior to hire and throughout employment
- Must pass a Criminal Background Check, local checks, and DCF Level II Background Screening prior to hire and annually
- Must have valid Florida Driver's License and pass motor vehicle checks prior to hire and annually with acceptable driving record maintained throughout employment along with reliable vehicle, valid registration, and valid vehicle insurance
- Ability to be flexible with hours to meet client scheduling needs
- Must have compassion for the clients we serve
- Must have knowledge of community resources
- Must have excellent clinical skills with a recovery orientation
- Must have strong time management skills
- Must have flexibility to meet the ongoing needs of our clients
- Must have a strong orientation to detail
- Must have excellent verbal and written communication skills
- Must have had experience working collaboratively with diverse groups of staff and community partners
- Must be able to demonstrate appropriate crisis intervention skills
- Must remain current in the general knowledge of psychotropic medication, side effects, and DSM IV diagnosis
- Must make a minimum of one monthly face to face contact, and one bi-monthly home visit, with all clients or documents reasons for failure
- Must meet productivity standard of at least 80%
- Must utilize support funds effectively to meet the needs of the clients
- Must complete and maintain documentation in a timely and accurate manner
- Must promote communication and collaboration among agency programs
- Must have Flexibility to provide services in client homes and other community settings
