Case Manager - Community Health

Case Manager - Community Health position in Southern Illinois supporting the SIRHSO program. Responsibilities include conducting initial participant intake, managing a caseload, facilitating community education/outreach campaigns, coordinating with a Registered Dietician, monitoring participant progress, addressing barriers to care (transportation, financial challenges), tracking referrals and outcomes, developing community partnerships, and ensuring continuity of care. Requires collaborative work with behavioral health/neighborhood resources, strong communication, organizational, and crisis-intervention skills, and the ability to work both independently and with a team.
