Job Summary
Case Manager for the Community Guardian Program assesses clients' needs and eligibility for in-home services and entitlements, and manages ongoing case work and care plans. Responsibilities include conducting intakes and screenings, providing accurate information on benefits and programs, assisting in accessing services through referrals and counseling, advocating with other agencies, applying and recertifying clients for entitlements, arranging for services, maintaining comprehensive case notes in the program’s data system, preparing required documentation and reports, and participating in supervision and staff meetings. The role requires respectful, patient communication with clients and families, potential supervision of social work students, and May involve translation services.
Required Qualifications
- Bachelor's Degree
- One year related experience and/or training
- Working knowledge of Microsoft Office Suite and other technology
- Excellent communication and listening skills
- Excellent customer service skills with a focus on treating clients with respect and dignity
- Able to multi-task and work independently with a great attention to detail
