Job Summary
Provide comprehensive case management, advocacy, and coordination for adults under court-appointed guardianship. Conduct initial assessments and monthly client visits, develop individualized service plans, coordinate healthcare/benefits/housing/legal assistance/home care, respond to crises, advocate for clients, and ensure compliance with court-ordered services. Maintain accurate case records, complete required documentation, and collaborate with community providers to support client well-being and stable outcomes.
Required Qualifications
- Bachelor’s degree in Social Work, Sociology, Psychology, Human Services, or a related field required.
