Job Summary
Manage an assigned caseload of program members in collaboration with an RN Case Manager by completing comprehensive assessments, member-centered care plans, and functional screens. Provide ongoing care coordination and quality assurance monitoring while maintaining required documentation in adherence to established standards. This role requires a Bachelor's degree in Human Services or related field, one year of experience with seniors or adults with disabilities, and eligibility for long-term care functional screener certification. Regular local travel using a personal vehicle is necessary. The position offers a hybrid weekday schedule from 8:00 AM to 4:30 PM with a $8,000 sign-on bonus. Community Care coordinates supportive services for over 13,000 Wisconsin residents to help them live independently within the community.
Required Qualifications
- Bachelor's Degree in a Human Services or closely related area
- Eligibility for long-term care functional screener certification
- Valid driver's license
- Proof of required insurance coverage
Desired Qualifications
- One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity
