Job Summary
Case Manager role focusing on effective case management for children and youth, ensuring compliance with DHS CUA Practice Guidelines. Responsibilities include developing youth-centered Single Case Plans, conducting home visits and assessments, reporting safety concerns, completing paperwork, maintaining confidentiality, and working collaboratively with support teams and agencies. Must possess a Bachelor’s degree in a relevant field, two years of applicable experience, and valid clearances. Evening and weekend work is expected, and a reliable vehicle is required for transportation.
Required Qualifications
- Bachelor’s degree in Social Work or related field including sociology, psychology, counseling, criminal justice, or education.
- Two years’ experience in Child Welfare or a related field.
- Valid, timely Child Abuse, Criminal History, and FBI clearances that meet all regulatory requirements.
- Good oral and written communications skills in English.
- Certification as a Direct Service Worker according to the requirements set out by the Commonwealth and the Office of Children Youth and Families in regulation and bulletin.
Desired Qualifications
- Basic familiarity with Microsoft Word, Outlook, and Excel.
- Ability to learn and effectively operate other software packages as needed and required.
