Case Manager - Co-Occurring Residential Program

Job Summary

Conduct case management needs assessments within 48 hours of admission and develop discharge plans to support client transitions. Assist clients with securing housing, employment, and public benefits while coordinating services with community, behavioral health, and legal partners. Meet individually with each client weekly to review progress on treatment plans and facilitate assigned groups including life skills and Seeking Safety. Provide crisis intervention services as needed and ensure clients meet legal obligations by submitting monthly progress reports. Maintain accurate client records and document all services on the same day they are provided. Participate in weekly treatment team meetings and the Management On-Call schedule as assigned.

Required Qualifications

  • High School Diploma or GED
  • Minimum of 1 year experience in one or more of the following: Residential or Outpatient Treatment
  • Minimum of 1 year experience in one or more of the following: Working with adults with mental illness / substance use disorders
  • Minimum of 1 year experience in one or more of the following: Social Services
  • Computer literate with familiarity of Microsoft Office
  • Grounded in your own recovery, if applicable, 1 year of sobriety or more
  • Ability to be firm and consistent, yet compassionate
  • Ability to work efficiently and to switch tasks effectively
  • Must work scheduled shifts
  • Reliability to fill shifts and support the team as needed
  • Willingness to accept supervision and direction
  • Ability to uphold professional boundaries, confidentiality regulations, agency policies and procedures
  • Interact in a professional manner with a diverse workforce, clients, and the public
  • Must be at least 21 years of age
  • Possess a current driver's license
  • Have a good driving record
  • Be insurable on the agency's liability policy
  • Knowledge of Mental Illness and Addiction
  • Communication skills to problem solving directly with clients and other staff
  • Teachable – Willingness to learn and grow in your position
  • Pass a pre-employment drug screen
  • Pass Utah DHS – Office of Licensing background screening
  • CPR and First Aid Certification or become certified in the first 30 days
  • Ability to walk up and down stairs
  • Ability to move around the facility, interacting with clients for extended periods
