Job Summary
Case Manager coordinates services and care for patients by providing coaching, education, and coordination of health and wellness resources to support recovery and whole-person care. You will develop, implement, and update treatment plans, connect clients with needed services and supports, and engage with patients in office, home, and community settings (including telehealth) to address behavioral and primary care needs. The role emphasizes trauma-informed practice and collaboration with clients and families to improve daily living and coping skills while documenting in the EMR and meeting productivity requirements.
Required Qualifications
- Bachelor’s Degree in psychology, social work or related field
- Valid driver’s license in good standing
- Experience with Electronic Health Record preferred
