Job Summary
Coordinate care for youth in psychiatric crisis by facilitating admissions, discharges, and seamless transitions to community-based services. Monitor individual treatment plans, obtain updates from service providers, and provide regular progress reports to families and discharge resources. Manage documentation, case records, and OMH reports while collaborating with the Program Manager and clinical team to determine admission appropriateness. Act as Campus Leadership for designated shifts, handling phone calls, overseeing medication administration, and ensuring staff coverage and youth accountability.
Required Qualifications
- Bachelor's Degree in a Human Services field
- Valid NYS driver's license
- Ability to sit, stand, walk, lift, carry, push/pull, climb, bend, and stoop
- Ability to perform fine motor skills
