Job Summary
Case Manager responsible for the development, implementation and management of all clinical aspects of Centre for Neuro Skills under the supervision of the Director of Rehabilitation. Roles include bridging clinical and administrative matters across clinic and residential sites; executing the six core components of clinical case management (processes and relationships, health care management, community resources and support, service delivery, psychosocial intervention, rehabilitation case management); requires full-time weekday hours 7:30am-4:30pm with some on-call/overtime; responsibilities include collecting patient information, planning objectives/goals/actions, coordinating with external stakeholders and families, implementing interventions, and evaluating plan effectiveness.
Required Qualifications
- Bachelors degree in a related field required. Masters or licensure/credential preferred but experience can be used to offset this level of education/licensing.
