Job Summary
Assess members' health and safety needs to develop comprehensive Member Care Plans and conduct face-to-face social assessments at the member's residence. Coordinate services with field care nurses to support frail elders and adults with disabilities in the least restrictive environment while arranging non-medical resources for social integration and housing. Maintain monthly contact via phone, perform quarterly in-person visits, and ensure cost-effective service delivery through accurate documentation of case notes and authorizations. Work Monday through Friday, 8:00 am to 4:30 pm CST, with up to 40% travel required to La Crosse County, WI. Valid driver's license and personal vehicle insurance are mandatory.
Required Qualifications
- Bachelor's degree in human services or a related field
- At least 1 year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities
- Valid state driver's license
- Proof of personal vehicle liability insurance with at least 100/300/100 limits
- Reside within 45 mins or less of the assigned coverage area
- Case Management experience
- Experience with electronic case note documentation
- Knowledge of community health and social service agencies and additional community resources
- TB Screening
