Job Summary
Provide comprehensive case management services to frail elders and adults with disabilities within Wisconsin Family Care, coordinating with a Field Care Nurse to deliver services in the least restrictive environment, conducting face-to-face social assessments at enrollment and periodically, performing quarterly in-person visits with monthly phone contact, arranging non-medical supports (social integration, housing, employment, community resources), ensuring cost-effective service delivery, evaluating risk, educating members, and maintaining accurate documentation (case notes, service authorizations, MCP updates); travel up to 40% within Juneau County and surrounding areas; work location includes Mauston and Baraboo, WI; remote/hybrid work possible with in-office interactions as needed; typical hours are Monday–Friday, 8:00 am – 4:30 pm CST.
Required Qualifications
- Bachelor's degree in human services or related field with at least 1 year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; OR Bachelor's degree in another field with at least 3 years of such experience
