Job Summary
Advocate for Veterans and facilitate access to housing assistance, utility assistance, healthcare, transportation, income support, and other community resources. Conduct home visits and community-based follow-up contacts to monitor housing stability and assess ongoing needs. Develop individualized housing and service plans in collaboration with participants, establish measurable action steps, and coordinate referrals to support self-sufficiency. Assist with completing housing and benefit applications, accompany participants to appointments, and document service delivery including housing inspections. Coordinate with the Lead Case Manager to review caseloads and discuss client progress, while actively participating in monthly community coalitions and stakeholder meetings. Maintain a comprehensive database of community resources and referral partners.
Required Qualifications
- Bachelor's Degree in Social Work, Sociology, Psychology or related field
- 1+ years case management experience
- Must maintain regular and acceptable attendance
- Driver's License with clear record
- Must have daily use of a vehicle without prior notice
- Must be available and willing to travel to various locations and with such frequency as the business need dictates
- Must be able to lift 50 lbs
Desired Qualifications
- 3+ years case management experience
- 2+ years in a customer service focused environment
- Experience with homeless, low income, veterans & their families
- LMSW, LBSW, LMFT
- Up to 50% travel within the county and surrounding areas
