Job Summary
Case Manager at Catholic Charities supports individuals navigating available services, conducts intake screening, completes and reviews intake documentation, performs housing office intake assessments, and develops/monitors ISP with semi-annual reviews to track progress toward housing and service milestones. The role requires documenting in the Uniform Case Record, coordinating referrals and entitlement documentation, and maintaining ongoing progress reports while working a Monday–Friday schedule. Required Qualifications
- Associate's Degree in Social Work
- 1–3 years’ experience in human services or related field
- Bilingual/Spanish preferred
- Valid Driver's License with access to a working vehicle
