CASE MANAGER / CAREER COUNSELOR

Job Summary
Conduct comprehensive participant intake, assessment, and eligibility determination for youth and young adults ages 14–24. Develop individualized Career Plans aligning documented skills and interests with viable education, training, and employment pathways. Establish measurable short-, intermediate, and long-term academic, career, and financial goals while monitoring progress and revising plans as needed. Provide ongoing case management, coaching, encouragement, and accountability to support successful completion of service plans. Identify barriers to success and coordinate interventions through internal and external referrals, advocacy, and supportive services. Maintain regular contact throughout program participation, including follow-up after placement. Deliver positive reinforcement, career coaching, conflict resolution, and motivational support. Maintain accurate documentation in required database systems (EmployRI/LACES) and coordinate engagement in activities ranging from resume development to apprenticeships.
Required Qualifications
- Bachelor's degree
- Experience working with youth and young adults, preferably ages 14–24
- Demonstrated ability to motivate, coach, and mentor individuals from diverse backgrounds
- Strong interviewing, assessment, documentation, organizational, and communication skills
- Ability to work independently while effectively managing a diverse caseload and meeting program performance goals
Desired Qualifications
- Experience in case management, career counseling, workforce development, education, human services, or a related field
- Experience working with at-risk youth and young adults, including individuals with learning differences or other barriers to employment and education
- Knowledge of community resources and workforce development systems
- Commitment to the mission and values of Tri-County Community Action Agency
