Job Summary
Case Manager - Care Management coordinates interdisciplinary, patient-centered care for individuals with acute and chronic health conditions, ensuring smooth transitions across the continuum, educating patients and families with health literacy best practices, and guiding care plans with focus on best-practice protocols, regulatory considerations, and efficient resource use. The role requires an RN with a BSN or higher (preferred) and at least two years of clinical nursing experience, with knowledge of care management models, CMS guidelines, Interqual criteria, and effective coordination across inpatient, ambulatory, home health, hospice, and long-term settings. Strong organizational skills, ability to engage patients and families in goal setting, and proficiency with electronic systems are essential.
Required Qualifications
- Current applicable state RN licensure
- BSN or higher preferred
- Case management certification preferred
- Two years clinical nursing experience required
- BLS for Healthcare Providers from the American Heart Association required for Oregon
