Job Summary
Care Management for High-Risk Pregnancy Case Manager coordinates education, referrals, and support for pregnant women with identified risk factors. Performs person-centered interactions in-person at Medical Home, via home visits, telehealth, or phone. Develops person-centered care plans and goals related to identified risk factors; conducts member education and follows up based on need. Coordinates service plans with other agencies; completes documentation in Virtual Health daily; makes referrals to CHA, ancillary agencies, and community resources. Education/experience options include a bachelor's degree in social work, a master's in social work, a BSN, or graduation from an accredited school of professional nursing with two years of experience (public health or maternity hospital experience preferred). Requires a valid RN license in North Carolina, a NC driver's license, and CPR certification. Knowledge areas include maternal and postpartum health, public health principles, community resources, and ability to engage diverse populations; strong documentation, communication, and teamwork abilities; adaptable to changing priorities.
Required Qualifications
- Bachelor's degree in social work, or a master's degree in social work, or a B.S. degree in nursing, or graduation from an accredited school of professional nursing with two years of professional experience (preferably in public health or hospital maternity setting)
- Valid license to practice as a Registered Nurse in North Carolina
- Valid NC driver's license
- Current CPR certification
