Job Summary
Conduct direct case management services including assessment, counseling, crisis intervention, referral, and advocacy to ensure an effective social service support plan is in place for families experiencing homelessness. Utilize the DHS CARES system to complete intake assessments, service plans, incident reports, and progress notes while reviewing information from Human Resource Administration and other agencies. Coordinate services to stabilize the family unit, address substance abuse, and improve housekeeping and money management skills to facilitate transition to permanent housing. Provide individual counseling and facilitate groups to enhance life skills and achieve self-sufficiency. Maintain accurate documentation of client interactions and progress in compliance with State, City, and Agency regulations. Serve on-call for a one-week period each quarter and travel throughout the New York City area.
Required Qualifications
- Bachelor's degree preferred and/or an Associated degree in Social Work, Sociology, Psychology, Counseling, Criminal Justice or Human Services
- Bachelor's degree (preferred) with a minimum of one (1) year of experience in social services, counseling, crisis intervention, family services, preventive services or housing services
- Associate degree (considered) with a minimum of two (2) years of experience in social services, counseling, crisis intervention, family services, preventive services or housing services
- Experience facilitating groups for homeless adults
- Specialized knowledge of substance abuse issues and treatment
- Family dynamics treatment
- Bilingual language ability highly desirable
- Excellent written and verbal communication skills
- Excellent interpersonal skills
- Computer literate in Microsoft-Office
- May be required to work long hours and a varied schedule
- Travelling throughout various locations in the New York City area
- Lifting and moving objects weighing up to 30 pounds several times a week
