Case Manager, C3 Skid Row

Job Summary
Provide intensive case management to highly vulnerable homeless individuals in Skid Row, engaging street-based clients, linking them to interim and/or permanent housing and supportive services, and coordinating with the C3 team to connect participants to health, mental health, housing, and other resources. Maintain accurate electronic records, perform needs assessments, provide information and referrals, support aftercare and follow-up, and collaborate with external agencies to remove barriers to housing while modeling wellness, recovery, and coping for program participants.
Required Qualifications
- BA degree or equivalent experience
- Demonstrated knowledge of homelessness, mental illness, substance abuse, medical issues, and domestic violence
- Willingness to work in a high tolerance Harm Reduction model with clients who have multiple barriers
- Working knowledge of social services agencies in Metro LA (SPA 4) and LA County agencies serving the homeless
- Excellent human relations/communication skills; ability to act professionally regardless of circumstances
- Skilled in non-violent crisis intervention
- Ability to self-motivate and organize time effectively and efficiently
- Computer literate; able to use computerized database system for information management
- Passion for working with the population served
- Ability to work as part of an interdisciplinary team
