Job Summary
Conduct clinical assessments and level-of-care determinations for individuals with Serious Emotional Disturbance or Intellectual/Developmental Disabilities. Develop individualized service plans, coordinate referrals and authorizations, and track progress toward goals in partnership with the multidisciplinary team. Maintain accurate documentation and provide advocacy and crisis response as needed. Work Monday through Friday from 8:30 am to 5:00 pm with some on-call responsibilities. This role serves as a unifying link between families, children, and community supports within the H-NNCSB service delivery system.
Required Qualifications
- Bachelor's degree in a Human Services field such as Social Work, Psychology, or Counseling
- Two (2) years of experience providing mental health case management or care coordination services for children
- knowledge of DSM-5 diagnosis
