Job Summary
Conduct community outreach through educational sessions, assessments, and interviews to prioritize health and social service needs. Help residents connect to resources by maintaining referral networks and coaching behavior change to achieve health goals. Travel extensively to community locations and partner agencies to build relationships with clients, providers, and institutions while reducing cultural and socio-economic barriers. Perform case management duties, attend team meetings, and participate in ongoing education to enhance skills. Respond to public health emergencies as needed. This role supports PABS's mission to create pathways to access, benefits, and a sustainable future for low-income New Yorkers facing federal cuts to SNAP and Medicaid.
Required Qualifications
- High school diploma or equivalent
- Associate or bachelor's degree in a related field
- Experience in community outreach, health education, case management, or social services
- Strong communication skills and ability to build trust with residents from diverse cultural and socio-economic backgrounds
- Comfort conducting field-based work and traveling locally to community sites and partner agencies
- Basic computer skills and ability to enter timely, accurate data into required systems
- Commitment to health equity, anti-racist practice, and resident-centered engagement
- Applicants must be authorized to work for ANY employer in the U.S.
Desired Qualifications
- Case Management and/or other relevant experience
- Knowledge of Bronx-based health and human service resources and referral pathways
- Experience supporting behavior change, coaching, facilitation or patient navigation
- Bilingual or multilingual skills aligned to community needs
