Job Summary
Medical Case Manager responsible for outreach, care coordination, and support services for individuals living with or at risk for HIV/AIDS. Duties include conducting outreach with community partners, connecting clients to medical care and social supports, developing individualized care plans, providing referrals and resource navigation, performing home visits and transportation to appointments, health education and interpretation as needed, documenting in CareWare/EHR, and collaborating with HIV Services teams on prevention strategies while staying current on harm reduction best practices. Requires strong communication and organizational skills, ability to work independently and within a multidisciplinary team, cultural competency with diverse populations, and willingness to learn phlebotomy. Residency in or familiarity with the Brockton area preferred; valid driver’s license and reliable transportation required. Evening/weekend flexibility may be needed.
Required Qualifications
- Associate or Bachelor’s degree in a human services field preferred
- Minimum of 2 years professional or volunteer experience in a related field required
- Case management or care coordination experience required
