Job Summary
Case Manager at Brandywine Counseling & Community Services (BCCS) provides assessment, evaluation, planning, linkages, monitoring, advocacy and treatment for clients dealing with substance use and/or mental health issues. Works in a cooperative team to provide assessment, coordination, and monitoring services to referred clients, completes Well Being Care Management assessments to inform an Individualized Care Plan, maintains a high rate of face-to-face client contacts, aids service linkage, monitors services, updates the EMR, and provides transportation and referrals as needed. This is a community-based role with regular travel outside of an office and up to 100% in-community work, requiring professional standards and ongoing client support.
Required Qualifications
- Bachelor’s degree in behavioral health or a related field
- 1 year experience in the Behavioral Health field
- 1 year Case Management experience
- Must have a valid driver's license with 2 points or less.
- Monday - Friday
- 7:00 a.m. - 3:00 p.m.
- From $21/hr. commensurate with experience
