Job Summary
Case Manager responsible for assessment, planning and coordination of transitional services for offenders re-integrating from a correctional facility to the community, ensuring access to clinical services or clinical placement upon release to promote continuity of care. Position requires a Bachelor’s degree (or equivalent experience) and two years in case management/assessments, with English/Spanish bilingual preference; shift options include day and evenings during weekdays. Located in Norwalk, CT as a full-time role within a community-based forensic health services program partnering with the Connecticut Judicial Branch.
Required Qualifications
- Bachelor’s degree from an accredited university or college; degree may be substituted with experience on a year-for-year basis
- Two (2) years’ experience administering assessments and performing case management functions
- Bilingual English/Spanish preferred
- Must be able to pass background investigation and obtain agency security clearance
