Job Summary
Assist individuals experiencing homelessness with housing applications, eligibility determinations, and placement strategies across HUD, DMHAS, and DOH programs. Negotiate with landlords and property managers to secure housing opportunities while conducting home visits, office visits, and telephone outreach for case coordination. Support participants in obtaining necessary documentation, developing self-advocacy skills, and maintaining lease compliance through regular progress reviews and program recertifications. Complete housing inspections, acuity indexes, and rent reasonableness reviews while collaborating with families, service providers, and community partners to promote participant success. Maintain accurate documentation in HMIS and EHR systems in accordance with regulatory requirements.
Required Qualifications
- High School Diploma or GED
- Associate's or Bachelor's degree in a Human Services-related field
- Bachelor's degree
- Minimum of two years of experience in human services
- Valid driver's license
- Reliable transportation
- Willingness to use a personal vehicle for agency business
- Must maintain an acceptable driving record
- Must be insurable at a reasonable rate under the organization's automobile insurance plan
- Proficiency in Microsoft 365 applications
- Ability to effectively navigate and utilize Electronic Health Record (EHR) systems
- Strong written and verbal communication skills
- Ability to work both independently and collaboratively as part of a multidisciplinary team
- Must be available for occasional evenings, weekends, and holidays
Desired Qualifications
- Associate's or Bachelor's degree in a Human Services-related field
- Experience working with individuals experiencing homelessness, housing instability, mental health challenges, and/or substance use disorders
- Knowledge of housing programs, community resources, and supportive service systems
- Experience assisting clients with housing applications, housing navigation, or case management services
- Proficiency with Microsoft Office applications, including Word and Outlook
- Experience utilizing the Homeless Management Information System (HMIS)
- Excellent time management, organizational, and prioritization skills
- Ability to effectively engage and work with diverse populations in a respectful, person-centered manner
- Highly motivated, creative, and resourceful with strong interpersonal and relationship-building skills
